Job description
We are seeking a compassionate and experienced Physical Therapist to join our home hospice team. As a Physical Therapist, you will work closely with our interdisciplinary team to provide comprehensive physical therapy services to hospice patients in the comfort of their homes. You will help patients manage their pain and improve their physical function, mobility, and quality of life in the end-of-life stage.
Responsibilities:
- Assess patient's physical capabilities, limitations, and needs, and develop an individualized physical therapy plan of care
- Provide hands-on physical therapy interventions, including exercise programs, manual therapy, and modalities to help patients manage their pain, improve their strength, endurance, balance, and mobility, and prevent falls and complications.
- Educate patients, families, and caregivers on safe and effective exercises, mobility techniques, and equipment use to promote independence and comfort.
- Monitor and document patient's progress, response to treatment, and adjust the plan of care as necessary to ensure optimal outcomes.
- Collaborate with other members of the interdisciplinary team, including nurses, social workers, chaplains, and physicians, to ensure coordinated care and support.
- Participate in team meetings, in-services, and educational programs to enhance knowledge and skills.
Requirements:
- A degree in Physical Therapy from an accredited program.
- A current state license to practice physical therapy.
- Minimum of two years of experience as a physical therapist, preferably in hospice, home health, or geriatric care.
- Knowledge of the principles and techniques of pain management, functional mobility, and fall prevention.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently, manage time effectively, and prioritize tasks.
- Availability to work on-call and travel to patient's homes within the service area.
- Valid driver's license and reliable transportation.

Pay: $58.00 - $62.00 per hour
