Newsome Home Health Care is rapidly growing and in need of skilled nurses. We are looking for full time, part time or fee for visit nurses.
Servicing: Lockport, Lemont, Homer Glen, Boilingbrook and surrounding areas
Reports To: Director of Nursing, in the absence of DON reports to supervising RN
Position Summary: Registered Professional Nurse provides skilled nursing interventions in the treatment of patient illness, rehabilitative needs, and preventative care. He/she utilizes a holistic approach in the provision of ongoing assessments, problem identification and specific therapeutic interventions, teaching, and training activities. The Registered Professional Nurse is responsible to apply professional knowledge and skill, in accordance with State and Federal guidelines and professional standards of practice, to facilitate problem resolution, and thereby, the achievement of individualized patient goals and outcomes.
Position Qualifications:
Must be a Registered Nurse with a public health, home health care, or hospital background.
Must have at least one-year clinical experience.
Car with required proof of insurance coverage and valid Driver’s License.
Maintains annual TB test, CPR certification and other required documents and submits them to HR.
Knowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentation.
Physical Qualifications:
Must be able to hear and speak in a manner understood by most persons.
Must be able to travel to prospective patient’s place of residence.
Able to assess and communicate will ill patients, co-workers, and public.
Able to work in a stressful environment.
Able to bend, stoop, and stand an average of 6 hours per day.
Must be able to lift and transfer patients.
Lift up to 50-75 pounds.
Duties and Responsibilities:
Demonstrates leadership and provides direction to members of the patient care team through case management of the patient home health episode of care. Case management of patient services includes:
Initial in-home evaluations and /or assessment.
Utilizes a systematic, individualized, goal/outcome driven approach in developing the nursing plan of care in the delivery of nursing services. Ensures proactive revision or addition to the plan of care to meet the patients’ needs as assessed or diagnosed, and as required by policy/regulation.
Collaborate with physicians, other agency staff and contract services to coordinate and implement the POC.
Communicates Laboratory test results to physician, obtains interim/verbal orders and communicates changes to the patient.
Appropriately communicates to ensure adherence to professional standards in the provision of and availability of supplies, materials, and equipment needed to implement the plan of care safely and effectively.
Documents direct patient services, coordination, and collaboration with physician and other disciplines or provides in the care of the patient, ensuring document is complete, and complies with acceptable home health standards and agency policy.
Demonstrates critical thinking skills by seeking resources for direction when needed, performing independent problem solving, logical, and deliberate decision making, exercises safe judgement, practices within legal and ethical guidelines, compares results of actions to desired goals.
Facilitates active and effective communication with team members as demonstrated through leadership of patient case conferences, staff education or in services, and timely clinical decision which provide guidance in the delivery of patient care.
Documentation is timely and meets professional standards of practice, establishes compliance with State regulations and Medicare conditions of participation.
Participate in case conference meetings.
Participate in monthly education.
Provides patients and staff supervision 24 hours a day when assigned on call.
Collaborating with leadership/management team and office staff to facilitate timely office processing of paperwork and personnel information.
Actively participates in Agency Quality Improvement activities.
Promote the Agency and services to the public and participate in physician and community education related to services provided by the agency as registered.
Performs other duties as assigned.
Maintains responsibility to develop professional self-attend seminars, identify self-learning needs, collaborates with agency leaders in meeting learning needs.
