Community Health Coordinator - Aiken, Barnwell, Clarendon, Edgefield, and Allendale Counties
Job description
This position serves as subject matter expert and supports Policy, System, Environmental (PSE) activities for nutrition education programs. It assists with creating new community outreach opportunities within designated counties. This position is classified as telecommuting, requires travel, and includes other duties as assigned.
JOB DUTIES:
30% - Essential - Health Promotion and Community Engagement: Identify and develop culturally appropriate partnerships to promote nutrition education and interventions for targeted audience. Formulate innovative approaches to expand engagement and outreach through community-building platforms in support of program initiatives. Participate in professional committees, school committees, community organizations, and regional- or state-level coalitions for networking opportunities. Coordinate PSE strategies to support direct education objectives. Serve as community expert and liaison between YLI and community-led initiatives within a designated region. Provide technical assistance and training in assigned program areas to staff and partners.
30% - Essential - Community and Public Health Interventions: Prepare, implement, and evaluate PSE strategies that support healthy eating and active living behaviors for low-income audiences. Develop PSE toolkits and provide related training to partners at the local, regional, or state level. Serve as a content expert in identifying appropriate strategies to adopt, provide technical assistance of implementation, and establish sustainable plans for PSE strategies. Assist Program Directors in expanding the reach of PSE strategies and evaluation efforts. Report PSE efforts and formulate entries using PEARS.
20% - Essential - Evaluation: Conduct participant evaluations using appropriate assessments. Evaluate survey results and organize assessment data accurately. Submit timely documentation of program evaluation data in accordance with federal and state guidelines. Review, evaluate, and report on assigned program areas.
20% - Essential - Administrative: Track expenses for materials, supplies and travel expenditures. Complete monthly time and effort documents. Submit accurate and timely expenses and effort reports. Maintain necessary program records for auditing purposes, including hard copies and electronic files. Other duties as assigned.
MINIMUM REQUIREMENTS:
Education - Bachelor's Degree in health education, health promotion or a closely related field such as psychology, biology, sociology, education or physical education and health education experience.
Licenses - Drivers License Class D normal < 26,000
Work Exp 2+ years
PREFERRED REQUIREMENTS:
Education - Master's Degree in nutrition, public health, dietetics, community nutrition,health promotion or a closely related field such as psychology, biology, sociology, education or physical education and health education experience.
