Duties:

• Formulate, evaluate and revise individual’s assessments, support plans and required reports.
• Ensure timely completion and distribution of required documentation.
• Schedule and facilitate support plan meetings.
• Maintain effective, efficient communication with families and funding agency’s personnel for assigned individuals.
• Communicate information regarding progress, changes and related issues to appropriate Health Center personnel in a timely manner.
• Instruct and monitor direct care professionals on goal development and documentation, and in the implementation of goals and behavior plans.
• Develop and maintain a database for goal creation.
• Participate in meetings as assigned, to include but not limited to, monitoring visits, team meetings, and medication reviews of assigned individuals.
• Participate in the admissions process as needed.
• Conduct all aspects of an assigned individual's initial orientation, inventory and required training.
• Maintain clinical records in accordance with established procedures, using MHC approved format.
• Develop and monitor implementation of planned program activities.
• Attend education meetings planned for the position.
• Any other duties as requested by Administrator.

• Some college with courses in Social Work preferred.
• Some experience working with the geriatric population; awareness of general conditions of residents in a LTC facility.
• Some Knowledge of medical terminology.
