Scope of Position: The Outreach Worker for the Mobile Engagement team provides intensive case management to homeless or those at-risk of homelessness through mobile community outreach work. The program is a rural outreach to the homeless and those vulnerable to homelessness in areas outside the Metro boundaries for the County. The Program Model includes Mobile Outreach, Relational Case Management and Community Networking. The primary objective of this position is to provide aid to individuals in housing attainment, utilizing the minimum assistance needed to address each household’s immediate housing crisis. The Outreach Worker will regularly visit and work with individual clients to identify and overcome barriers to housing and healthcare and refer these households to other resources in the community (mainstream services, benefit services, health care, food assistance programs, childcare resources, etc.).

EDUCATION AND WORK EXPERIENCE:

Minimum High School Diploma or GED; Associates degree strongly preferred. In lieu of degree, 2+ years of relevant experience with 2 years specific to work in the nonprofit realm. 2+ years demonstrated experience in related social services is preferred.

LICENSE AND CERTIFICATIONS:

Knowledge, Skills and Abilities Required:

  • Understand and comply with mandatory reporting requirements and laws pertaining to vulnerable adults and children.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Follow a prescribed schedule to reach the different pockets of homeless in Washington County with coordination of other agencies and the Metro County area.
  • Support the maintenance of a Master service and referral information list of partners within Washington County and work with other Outreach teams in the area to update and maintain.
  • Conduct initial assessment to determine client’s specific needs. Meet with clients as defined, developing rapport.
  • Maintain weekly contact with clients while they are working on their housing assessment and housing readiness plan. Include referral and linkages for employability, job readiness, and vocation education and training needs to help obtain client economic and social self- support.
  • Provide intensive housing assistance including all or part of a comprehensive assessment, case management, service authorization, information, and referral necessary for adults to develop and pursue an effective and mutually agreed upon housing plan.
  • Provide expert case management for clients with varying degrees of vocational, social, cultural, and/or medical impairments hindering self -sufficiency.
  • Post housing case management may include referrals and linkages that assess and determine employability, job readiness, and vocational education and training needs and client economic and social self-support.
  • Provide reports on success of program.
  • Become fully trained on the Community Connect Assessment and refer properly.
  • Provide formal and informal referral service to community resources, case management services in a multidisciplinary approach to service delivery.
  • Participate in appropriate committees and agency meetings in the absence of the Program Manager.
  • Coordinates services with other agencies such as mental health, Employment Security, substance abuse programs, vocational rehabilitation, and developmental disabilities.
  • Participate in unit staff meetings and in applicable training as assigned.
  • Maintains client case records, social services system (Wellsky), HMIS, and ensures required forms and reports are maintained in the prescribed format.
  • Track, analyze, and input data relevant for county, donor, and internal stakeholders.
  • Must be willing to travel/drive throughout rural WA County using organizational vehicle. Schedule to be developed in partnership with the O/E Program Manager.
  • Meet all health and safety regulations as defined by the Health Department as well as The Salvation Army.
  • Performs other administrative and operational functions as requested.

PHYSICAL REQUIREMENTS:

Reaching – extending hands and arms in any direction.

Fingering – picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand as in handling.

Pulling – using upper extremities to exert force to draw, haul or tug objects.

Talking – expressing or exchanging ideas by means of the spoken word. Those activities in which they must convey detailed or important spoken instructions to others accurately, loudly, or quickly.

Hearing – perceiving the nature of sounds at normal speaking levels with or without correction. Repetitive motion – substantial movements (motions) of the wrists, hands, and/or fingers.

Driving or riding in a motor vehicle at night and in all types of weather conditions encountered in work locales.

Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of

force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.

Close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading. Visual acuity to operate motor vehicles.

ENVIRONMENTAL CONDITIONS:

Work outside in various weather conditions.

Interact with the individuals who may struggle with aggressive behaviors.
