BrightStar Care of Scott and Carver Counties is seeking an Occupational Therapist (OT) to work with our Home Health Care clients. Our clients are residents of Scott and Carver Counties, many of which are Veterans. Position Responsibilities:

· Provide occupational therapy services to clients according to physicians’ written orders.

· Assess and evaluate therapeutic/rehabilitative/functional status.

· Develop a plan of care and treatment making sure the plan of care is consistent with the assessment.

· Assess the home environment specifically as it relates to fall risk and interventions and home safety.

· Report client’s response to treatment or changes in condition to DON and physician.

· Instruct other personnel and/or family members on goals.

· Assist in identifying client and family/caregiver needs for other services.

· Prepare and submit clinical progress summaries based on identified and attained goals.

· Supervise Occupational Therapy Assistant(s) in accordance with organization policy and state regulations.

· Assure compliance with all local, state, and federal regulations.

Required Qualifications:

· Graduated from an Occupational Therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE)

· A Minnesota State Licensure

· Two years of appropriate experience as an Occupational Therapist, home health care is preferred.

· Ability to communicate with physicians and medical staff regarding patients care and current status.

Pay: $27.79 - $38.00 per hour
