Essential Job Duties

1. Provides technical nursing services in accordance with agency procedure manual policies and Licensed Practical Nurse licensure regulations.

A. Assists the registered nurse in performing specialized procedures.

B. Prepares equipment and materials for treatments, observing aseptic technique as required.

2. Prepares clinical and progress notes.

A. Submits timely and accurate documentation of each nursing visit.

3. Assists the patient in learning appropriate self-care techniques.

4. Confers with case manager or clinical supervisor regarding changes in patient’s condition and needs.

A. Reports on each patient visit to the case manager or clinical supervisor.

B. Participates in case conferences as needed.

C. Implements strategies to prevent hospitalizations or re-hospitalization.

5. Attends in-service programs.

6. Maintains current C.P.R. Certification

7. Participates in civic and community health activities as requested.

8. Accepts responsibility under adverse working conditions such as inclement weather conditions or substandard living conditions of patients, etc.

9. Assumes responsibility for personal and professional growth.

10. Is knowledgeable regarding agency policy, procedures, and general information.

A. Attends selected workshops, staff meetings, and seminars to enhance job performance & knowledge of services.

B. Reports to work at the scheduled time and uses time efficiently.

C. Willingly performs other duties outside of job responsibilities as may be required.

Education, Training & Experience

1. Must have graduated from an approved school of Practical Nursing.

2. Must be licensed to practice in the State of Indiana.

3. Must have one year of nursing experience.

4. Licensed Practical Nursing curriculum must have included a pharmacology course.

Tools & Equipment

General office equipment, equipped Nursing bag, textbooks, teaching aids, periodicals, computer, cell phone, and pager.
