This is a full time hourly position. Monday-Friday 8am-5pm. The position offers mileage reimbursement, self scheduling for weekly visits and on call rotation. The coverage area is a team approach with geographic areas in Kerr, Bandera, Kendall, Medina, Kimble, Edwards, Real and Gillespie County.

The RN Case Manager will direct and specialize in end of life nursing care for hospice patients in their place of residence, coordinating care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for the coordination of care. Serves as a liaison between physicians and other service providers and patients and families. The age of the patient will be taken into consideration for age specific care.Required Education:
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Required Technical Skills:
Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Required Licensure /Registration /Certification:
Current Texas State license as a Registered Nurse, CPR certified, and current Texas Drivers License and auto insurance. Must have reliable transportation.
Required Experience:
Two years experience as an Registered Nurse with med/surg experience.
