GIL Foundation is seeking a qualified Registered Nurse to join our established team. The right RN candidate should have an attention to detail, be highly dedicated, detailed, organized, compassionate, and dependable. The Registered Nurse will supervise the nursing care needs of 15-20 adults with disabilities across 3-4 group home programs in North Haven and Cheshire. The residents of these programs are truly a special group and wonderful to serve. The guardians are very supportive of GIL and our staff. GIL is a private nonprofit organization seeking the right Nurse to provide supports and services for individuals with intellectual and developmental disabilities. GIL “Grounded In Love” has been committed to the highest standards of quality care and support in the community since 1985. We are currently recruiting a Nursing Care Manager RN (Full Time). The right candidates should have an attention to detail while also being dedicated and compassionate. The position include training and medication administration oversight for Direct Support Professional staff. These positions also offer flexible scheduling and diverse responsibilities in a great work environment. Laptop, cell phone, and mileage reimbursement offered. Collaborative Oversight and Support from the Director of Nursing.


The Essentials – What We Are Looking For:

  • This Nurse Care Manager for GIL should be a person of integrity who is highly committed, very responsible, and is organized with a definite attention to detail – considering the daily program assignments and compliance to state licensing regulations at this facility. A Nurse who is honest, faithful, and trustworthy; and demonstrates a dedicated work ethic will fit well in our company culture.
  • This Nurse Care Manager should also be caring and compassionate with respect to our residents and a solid coach and mentor for the staff members for which he or she will be responsible to supervise. A Register Nurse who is kind, patient, reasonable, and flexible with team members; and also good at observing for other's needs, while listening to others, and making room for efficient and effective mentoring and training within a diverse workforce.
  • This Nurse Care Manager will have flexibility with your schedule and availability due to the nature of 24-hour care for individuals requiring high levels of support. We do however provide and contract with after hours, weekend, and holiday on-call nursing for triage care and consultation to support our nursing team. Someone who is coach-able, interested in personal development, eager to learn and willing to ask questions will fit well in our company culture.

Essential Functions & Responsibilities (including but not limited to)


  • Provides for the ongoing assessment of medical care and related documentation to ensure it meets State of CT DDS and other regulatory requirements by developing and implementing plans for data management and collection, nursing directives, and quality review systems.
  • Assessing, coordinating, implementing, and monitoring healthcare plans and services for individuals with intellectual and developmental disabilities.
  • Ensures that medical and nursing plans are reviewed and revised using supporting data periodically and as needed; and that all subsequent reports are completed and reviewed timely.
  • Completes documentation, summaries, medication history plans, charts, etc., as required for proper program implementation and for Program Review (PRC), and Human Rights Committees (HRC).
  • Reviews, records, and maintains all required records, documentation, and data in a timely fashion; including but not limited to: Nursing Assessment, Self-Medication Assessments, Nursing Quarterly and periodic Reviews, Medication Administration, Healthcare Assessment, and Referrals.
  • Facilitates the exchange of information related to Support Services (OT, PT, SLP, RD, Behaviorist, etc.) working with planning and support team members, program managers, psychiatrists, consulting professionals, and direct support staff, as indicated for each individual.
  • Contributes with other clinical staff in the development, implementation, and ongoing revision of individual plans.
  • Actively attends and participates in IP meetings, planning and support team meetings and other relevant meetings, including staff meetings as needed.
  • Trains and assists staff in acquiring the skills needed regarding the medical needs, supports, and plans of program participants; and monitors and supervises for appropriate implementation.
  • Collaborates constructively with managers, clinical, and direct support staff to discuss concerns, activities, and outcomes for individuals; and reviews, revises, updates, and modifies strategies as needed.
  • Monitors, implements, and trains the medication administration procedure as it relates to the program.
  • Schedule and facilitate doctor appointments, treatments, and labs to assist Program Management as needed
  • Liaison, providing support, advocacy, and consultation to families, ancillary agencies, physicians, and other professionals.
  • Travels locally as necessary from site to site to provide direct and indirect services by observing participants, and monitoring the program and staff in the implementation of medical supports strategies in Prospect and Cheshire.
  • Delivers supports and services with care, compassion, and the highest standards of ethics and professionalism.
  • Flexible and subject to call for consultation, responding to emergencies, and providing solutions and strategies when needed with respect to the medical and clinical needs of program participants.

Specific Job Skills & Requirements:

  • Current CT RN License in good standing
  • Experience with ID/DD population or DDS (Department of Developmental Services) is preferred but not required
  • Intermediate computer skills are necessary - Outlook, Word, Excel, Therap
  • Diligent, responsible, and highly organized with attention to detail and follow through
  • Excellent and effective verbal, written, and interpersonal communication skills
  • Ability to manage multiple priorities, anticipate and identify problems/needs, and to recommend and execute appropriate solutions
  • Demonstrates necessary professional and technical skills and authentic trend and industry knowledge; while possessing the desire to learn and grow in this community
  • Maintains confidentiality of all resident and personnel performance, records and related documentation. This includes the accountability and safekeeping of all such information in accordance with agency policy, federal (HIPAA) and state regulations
  • Positive attitude anchored by integrity and an outstanding work ethic; and most importantly the desire to serve individuals with special needs in a nonprofit environment
  • Must successfully complete all pre-employment background checks and have valid CT Driver’s License
