Incumbent will partner effectively with young people to support the transition from foster care to adulthood in developmentally appropriate and identity affirming ways; facilitate enrollment and ongoing participation in the Fostering Connections program; implement the coaching approach practice model to support growth across life domains (housing, physical and mental health, daily living skills, employment and finances, education, supportive relationships and community connections, cultural and personal identity; parenthood); serve as an partner, advocate, and support to young people in internal and external staffings, meetings, and with community-based providers; ensure opportunities to participate in normative activities that build (life) skills needed for successful adulthood. Provide technical assistance to the field, foster/adoptive families, providers, and the community regarding youth and emerging adult development, the impact of trauma, and best practices to support engagement and thriving; provide technical assistance regarding the Chafee Act, NM Older Youth Services, and other legislation/policy/procedures that impact work with older youth.

THIS POSITION WILL COVER SANTA FE AND RIO RANCHO; THEREFORE, INCUMBENT MAY BE HOUSED IN EITHER LOCATION.

Minimum Qualification

Master's Degree in Social Work from an accredited college/university and two (2) years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health and/or health care, and licensure by the NM Board of Social Work Examiners at the LMSW or LISW level or eligibility for such licensure in accordance with NM requirements OR a Bachelor's Degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice or Family Services from an accredited college/university and four (4) years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health and/or health care. Any combination of education from an accredited college or university in a related field and/or related experience totaling eight (8) years may substitute for the required education and experience. Employment Requirements

Must possess and maintain a valid Driver's License. Pre-employment background investigation is required and is conditional pending results.

Working Conditions

State of New Mexico employees may be impacted by the Public Health Order dated December 19, 2022, which requires facilities licensed or certified by the Centers for Medicare and Medicaid Services ("CMS"), including all hospital types, long-term care facilities, nursing homes, hospice facilities and rehabilitation facilities, as well as assisted living facilities and adult day care settings, to adhere to all COVID-related requirements prescribed by CMS, including, but not limited to masking and staff vaccination.

Work is performed in an office setting: late hours, weekends, on-call and callback work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel is required. Possible exposure to irate clientele. Incumbent will work under stress and frequent time constraints. Incumbent will be required to furnish a personal vehicle for carrying out assignments which may include transporting clients including children. Must have own mode of transportation.
