As a Community Health Promoter II, you'll create a positive Scripps Health experience for our patients while being responsible for the following:
- Implementing community outreach and facilitating a variety of community-based education.
- Conducting a variety of community-based presentations which will be held in local venues.
- Following up with participants.
- Developing educational materials together with the Community Benefits team.
- Providing patient support and education which will be offered post discharge to link patients to needed social and medical supportive services.
- Managing documentation and evaluation forms as well as maintaining a list of community contacts and referrals
This is a benefitted Full Time, Day Shift position with a hybrid work schedule, located at Scripps Mercy Hospital Chula Vista.
Required Qualifications:
- High school diploma or equivalent.
- Excellent communication skills. Demonstrate oral and written communication skills by clear and accurate correspondence, including spelling and grammar usage.
- Demonstrate experience and ability to work independently and as a team player. Ability to set priorities and goals without specific direction. Ability to handle problems, propose solutions, and implement them.
- 1-2 years community-based work experience.
- Verbal and oral presentation skills.
Preferred Qualifications:
- Bilingual in Spanish.
- Basic typing and computer skills.
